Evolution Physical Therapy is a progressive, state-of-the-art practice that is expanding the scope of our profession to include not only exceptional, research-based rehabilitation treatment approaches but also fitness and wellness, preventative services, on-site sport movement screens, and corporate wellness plans.

We are preferred providers with most PPO insurance plans. With a physician's precription, services are covered by insurance and the financial responsibility is based on the individual plan of the patient. As a courtesy to patients, we will verify your insurance before your first visit, but we also strongly encourage all patients to verify their benefits with their insurance provider before treatment.
Cash rates for treatment (includes all services rendered)
Evaluation/first treatment = $150
Each subsequent treatment = $120
