New Patients
We treat a wide range of patients. All types of patients are treated with the absolute best in physical therapy care. We can help you. For your first visit with us:
- Your initial evaluation should take approximately an hour to an hour and a half. Please arrive 15 minutes early for your first visit to complete the necessary paper work.
- Filling out all your paperwork prior to your first appointment will speed up the check in process.
- Please click and download the paperwork on the right (please make sure to download the new patient paperwork as well as your specific body part that we will be treating)

On your first visit to physical therapy, please be sure to bring:
- Your driver's license or identification card
- Your prescription for physical therapy
- Your insurance card
- Comfortable Clothing
- Expect subsequent treatments to last approximately one hour. We provide shower facilities and steam rooms for all patients. Co-pays are due at time service is provided.
PLEASE NOTE - We strictly enforce a 24-hour-notice cancellation policy
It is the responsibility of each patient to verify his/her physical therapy benefits with their insurance companies prior to initiating care. As a patient courtesy, we will do our best to confirm coverage with each patient’s insurance plan, but that information is not a guarantee of benefits.
