On your first visit to physical therapy, please be sure to bring: - Your driver’s license or identification card - Your prescription for physical therapy - Your insurance card Your initial evaluation should take approximately an hour to an hour and a half. Please arrive 15 minutes early for your first visit to complete the necessary paper work. Or you can download the paperwork and fill it out prior to coming. You should plan to be at each visit for approximately one hour. We look forward to seeing you soon! |
